You may choose Only the items you desire.However, any funeral arrangements you select will include a Charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement.We provide describing the funeral goods and services you selected.

| A. Basic Services of Funeral Director and Staff, Including Overhead |

Our fee for the services of funeral director and staff includes, but is not limited to,staff response to the initial request for service; arrangement conference with family or responsible party; arrangement of funeral; preparation and filing of necessary local authorizations and
permits; recording vital statistics; staff assistance prior to,during, and following the funeral,including cemetery, crematory, and others. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance, and utility expenses; clerical and administrative costs; and equipment and inventory expenses.This fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select, and is included in our charges for cremation, immediate burials, forwarding and receiving remains, and our funeral packages.

I. Accommodations
Certified copies of death certificates (for Los Angeles County only) are processed online through "Vital Chek." Each certificate order will include a $6.00 charge for Vital Chek that will be added to your contract. The cost of death certificates is $16.00 each for the first 10 certificates and $16.50 for each certificate over ten (10).

Angelus Funeral Home accepts the following methods of payment: Cash, bank money order, cashier's check, personal checks, uncontestable life insurance policies, Visa, Discover, MasterCard and American Express.

Life Insurance
Families choosing to use life insurance to pay for funeral services and merchandise will be assessed an insurance processing fee on the total amount of insurance proceeds being used. Angelus Funeral Home does not receive any portion of this fee. Angelus uses the services of an insurance processing company to quickly process insurance claims and make funds available within the payment schedule established by the mortuary. In order for the insurance processing company to process these claims, the processing fee will be included on your contract as a Cash Advance item because it is money that is paid directly to the insurance processing company and not paid to the mortuary. If you choose not to pay these fees, Angelus Funeral Home will not accept your policy to pay for the funeral services and an alternative acceptable method of payment for services will be established.

CREMATION

Cremation Policy
Maximum weight allowance at Angelus Rosedale Crematory is 300 lbs. The services of an outside crematory facility will be used for cremations over 300 lbs. Transportation fees and additional weight fees per pound will be quoted prior to contracting. All containers for cremation must be constructed of hardwoods, composite material and/or heavy cardboard. All remains received for cremation must be encased in a rigid, combustible container. |

Many funeral homes require embalming if you're planning a viewing or visitation. But embalming generally is not required if the body is buried or cremated shortly after death, however some states do require embalming by law.
Under the Funeral Rule, a funeral provider:
- May not provide embalming services without permission.
- May not falsely state that embalming is required by law.
- Must disclose in writing that embalming is not required by law, except in certain special cases.
- May not charge a fee for unauthorized embalming unless embalming is required by state law.
- Must disclose in writing that you usually have the right to choose a disposition, such as direct cremation or immediate burial, that does not require embalming if you do not want this service.
- Must disclose in writing that some funeral arrangements, such as a funeral with viewing, may make embalming a practical necessity and, if so, a required purchase.
